These notices shall satisfy two separate but related procedural requirements for activities to be undertaken by Jackson County Board of County Commissioners.

REQUEST FOR RELEASE OF FUNDS
Chipola Quick Care/Chipola Wound Care Center
On or about June 01, 2012 Jackson County will submit a request to the Florida Department of Economic Opportunity (DEO) for the release of Community Development Block Grant funds under Title I of the Housing and Community Development (HCD) Act of 1974, as proposed, to undertake a project in relation to the Chipola Quick Care/Chipola Wound Care Project Scope: The following is a summary of the CDBG funds requested.  All of these activities are located within the existing jurisdictional limits of Jackson County and to be served by utilities owned by the County.  All of the CDBG funded activities will be located within public right-of-way, public property, or public easements.

Street Improvements
Jackson County is proposing to construct street improvements to serve the proposed site.  These improvements consist of two activities, a new public access roadway and improvements to the Intersection of US 90/SR 71.  The following is a description of those activities:

New Access Roadway:  Jackson County shall construct a new public access roadway from US 90 at the intersection of SR 71 north approximately 400 L.F.  These improvements shall include roadway pavement, street lighting, sidewalk, curbing, signage/pavement markings and a public bus stop for the local public transit system that will serve the facility.  The public transit system (JTrans) serves many local beneficiaries on Medicaid and Medicare so they may have access to doctor’s offices, grocery stores, and other facilities in Jackson County.  Cost of this activity is $146,480.00

US90/State Road 71 Intersection Improvements:   Jackson County shall construct improvements to the intersection of US 90/State Road 71.   These improvements include but not limited to roadway pavement, construction of an approximate 300 L.F. turnlane on US 90, curbing, mast arm and signalization, electrical, and pavement/markings.  These improvements shall occur within the intersection and extending approximately 500 foot in each travel direction.  Cost of this activity is $177,162.00
Total Cost of All Street Improvements:  $323,642.00

Natural Gas
Jackson County is proposing to construct improvements to the existing natural gas transmission system to serve the proposed site.  The facility will require central natural gas services.  Natural Gas Lines will be extended from current terminations on the south side of US 90 near Commercial Park Drive to the proposed site, 1250 L.F. to the east.  This will include natural gas lines, directional bore under highway, valves, meters, and appurtenant components. The related CDBG cost to this activity is $54,000.00.

Flood and Drainage Improvements
There is need to construct offsite storm water facilities for the development of the new medical facility. This will include construction of new public stormwater facilities that will serve the new facility and the new roadway improvements.  This facility will be located just to the east of the proposed development on the north side of US 90, on dedicated public stormwater retention easements. The related CDBG cost to this activity is $130,400.00.

Jackson County will also have additional infrastructure needs including Waterline and Sewer Improvements to support the facility.  These activities shall be funded by Rural Infrastructure Funding:

Water Line Improvements
Jackson County is proposing to construct improvements to the existing central waterlines system to supply water to the proposed site.  The facility will require water services.  2200 L.F. of Waterlines will be extended from current terminations just to the west of
the proposed facility on the north side of US 90 in the vicinity of Commercial Park Drive, to the proposed development site. This will include valves, meters, fire hydrant assembly, waterlines, and appurtenant components.
The related RIF cost to this activity is $77,500.00.

Sewer Facilities
Jackson County is proposing to construct improvements to the existing sewer transmission system to serve the proposed site.  The facility will require central sewer services.  Sewer Lines will be extended from current terminations on US 90 just to the west of the proposed facility on the north side of US 90 in the vicinity of Commercial Park Drive, 850 L.F. to the east.  This will include sewer lines, manhole, lateral connection, and appurtenant components.  The related RIF cost to this activity is $34,100.00.

Engineering
Jackson County is proposing to use RIF funding for all engineering costs related to the project.  This will include all design, permitting, Construction Inspection, and any other additional services related to the project.
The related RIF cost to this activity is $98,000.00.
Complete funding for the project being requested is as follows:
DEO-CDBG:    553,042.00
DEO-RIF:    209,600.00
Total Funds:    762,642.00

There are no wetlands or sensitive lands within the existing easement.

FINDING OF NO SIGNIFICANT IMPACT
Jackson County has determined that the project will have no significant impact on the human environment. Therefore, an Environmental Impact Statement under the National Environmental Policy Act of 1969 (NEPA) is not required.  Additional project information is contained in the Environmental Review Record (ERR) on file at the Jackson County Administration Office, 2864 Madison Street, Marianna, Florida 32448 and may be examined or copies requested weekdays 9:00 A.M to 3:00 P.M.

PUBLIC COMMENTS
Any individual, group, or agency may submit written comments on the ERR to Jackson County Board of County Commissioners. All comments must be received within 15 days following the publication date of this notice by May 31, 2016.  Comments will be considered prior to Jackson County requesting a release of funds.  Comments should specify which notice they are addressing.

RELEASE OF FUNDS
Jackson County certifies to the Florida Department of Economic Opportunity and HUD that Pam Pichard in his/her capacity as Interim County Administrator consents to accept the jurisdiction of the Federal Courts if an action is brought to enforce responsibilities in relation to the environmental review process and that these responsibilities have been satisfied. The State’s approval of the certification satisfies its responsibilities under NEPA and related laws and authorities and allows Jackson County to use the CDBG funds.

OBJECTIONS TO RELEASE OF FUNDS
DEO will accept objections to its release of funds and that Jackson County certification for a period of fifteen days following the anticipated submission date or its actual receipt of the request (whichever is later) only if they are on one of the following bases: (a) the certification was not executed by the Certifying Officer of Jackson County; (b) Jackson County has omitted a step or failed to make a decision or finding required by HUD regulations at 24 CFR part 58; (c) the grant recipient has committed funds or incurred costs not authorized by 24 CFR Part 58 before approval of a release of funds by the State; or (d) another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental quality. Objections must be prepared and submitted in accordance with the required procedures at 24 CFR Part 58, Sec. 58.76 and shall be addressed to the Florida Department of Economic Opportunity, CDBG Program, 107 E. Madison Street, Tallahassee, Florida 32399-4120.  Potential objectors should contact Jackson County to verify the actual last day of the objection period.
